I refer to your email dated 3 September 2019 to the Education Review Office requesting under the 
Official Information Act for a copy of the inquiry conducted by the Education Review Office to the 
death of Toran Henry from Takapuna Grammar in 2009. 
 
EROs main function is to review the performance of schools and early childhood services in the

ERO does not inquire into the deaths of individuals. I am therefore refusing your request under
