Please provide any documentation, correspondance, notes, meeting minutes, etc 
regarding; 
* Investigations as to whether other DHBs had sufficient standards, protocols, etc that
would avoid this happening in their DHB
* Whether any corrective actions were made at the other 19 DHBs as a direct result of this
case.”
