Official information request 
I am writing to acknowledge your official information request regarding the Family Violence Death 
Review Committee. We received your request on 23 August 2021 for the following: 
“May I kindly request the full meeting minute of this Committee from 1st Jan 2020 to now. Also, 
May I kindly request the pay rate of each members (how much they been paid per day or is it a 
hourly rate?) and any other expense reimbursements that each member received since 1st Jan 
2020.   
Also, to ensure transparency, I would like to encourage the Committee to consider published their 
full meeting minutes on their website for public to review.” 
We will endeavour to respond to your request as soon as possible and in any event no later than 
20 September 2021, being 20 working days after the day your request was received. If we are 
unable to respond to your request by then, we will notify you of an extension of that timeframe. 
Your request is being handled by the Mortality Review Committee. If you have any

Please note that the Commission publishes some of its OIA responses on its 
website, after the response is sent to the requester. The responses published
