I refer to your email of 9 December 2021, to WorkSafe New Zealand (WorkSafe) via FYI.org.nz, 
requesting the following information: 
 
•  I am writing to request a copy of each notifiable incident report submitted to Worksafe by 
GOLEMAN GROUP HOLDINGS LIMITED (2194601) since 31 December 2015. 
 
Your request has been considered under the Official Information Act 1982 (OIA).  
 
We have interpreted your request to be for a copy of each notification form submitted to WorkSafe 
by Goleman Group Holdings Limited since 31 December 2015. We can confirm there were three 
notification forms submitted by Goleman Group Holdings Limited which we have attached in 
answer to your request. 
 
Some information has been withheld from the information being released to you under section 
9(2)(a) of the OIA, to protect the privacy of natural persons. As per section 9(1) of the OIA, we do 
not consider that, in the circumstances of this case, the withholding of the information is 
outweighed by other considerations which render it desirable in the public interest to make the 
information available. 
 
You have the right, by way of complaint under section 28(3) of the OIA, to request a review of this 
response by the Ombudsman. Information about how to make a complaint is available at
