groups at the commission.
1. Can you please provide the database mentioned above (consumer forum
database), the name of organization would be fine?
The Consumer Health Forum Aotearoa (CHFA) database is run and operated by He Hoa
Tiaki | Partners in Care at the Health Quality & Safety Commission
2. Can you please provide the name of targeted organisations you have
approached?
Rainbow Youth, Te Pou, DHB Consumer councils, HQSC consumer network group and
consumer advisory group, Salvation Amy, St Vincent de Paul, University of Otago,
University of Waikato, Ara institute of technology, Te Hou Ora Whānau Services, Otago
Youth Wellness Trust, Porirua Niuean Community, Oranga Tamariki, MSD, Dunedin City
Council, Te Runganga o Ngai Tahu, Mana Pasefika, Vaka Tautua, Kaipatiki Trust, Arai Te
Uru Whare Hauora.
3. How do you come up with this database and when do you last update it? Would
that be avenue for general public to join this list/database?
The Consumer Health Forum Aotearoa (CHFA) database was established as part of the
new health reforms. Anyone can join the forum database at any time. It is continually
updated. Information about CHFA and how to join can be found:

Please note that the Commission publishes some of its OIA responses on its website,
after the response is sent to the requester. The responses published are those that are
