Thank you for your email of 9 September 2023 to Oranga Tamariki—Ministry for Children
(Oranga Tamariki), requesting the actions taken regarding the recommendations from
Dame Karen Poutasi and the Office of the Chief Social Worker following the death of
Malachi Subecz. Your request has been considered under the Official Information Act
1982 (the Act).
You have requested:
Please provide the actions taken regarding each of the 14 recommendations from
the external review by Karen Poutasi into Malachi's death - I understand if this wil
require partial transfer to another agency Please provide Oranga Tamariki's
actions regarding each of the recommendations made by the Office of the chief
social worker into Malachi's death
We are currently working to proactively release information regarding Dame Karen
Poutasi’s review and recommendations, and of the actions undertaken by Oranga
Tamariki and other agencies in response to this. We hope to have this information
published soon. We recommend that you check the ‘Reviews and inquiries’ page on our
website for more information when this is published:
