We  refer  to  your  request  under  the  Official  Information  Act  1982,  dated  14  July  2015  requesting  the 
following information. 
 
  All communications, meetings and dialogue between Pacific Trust Canterbury and the Canterbury 
District Health Board between 1st of July 2014 and 2nd of July 2015, in regards to mental health 
services, its funding, reports and all communications during this time. 
 
Please refer to Appendix One (attached) for all correspondence between Canterbury District Health 
Board and Pacific Trust Canterbury from 1/7/2014 to 2/7/2015 regarding mental health services. 
 
We are declining to include Mental Health Additional Reporting MHP-63C – Community Services for 
Pacific People (Adult/Youth) under section 9(2)(a) of the Official Information Act 1982. 
 
“9(2)(a) protect the privacy of natural persons, including that of deceased natural persons;”  
There were a number of face to face meetings and telephone conversations that did not have minutes 
recorded but the outcome of these is reflected in the correspondence.
