Thank you for your request under the Official Information Act (1982), dated 16 July 2015 and received 
by the Canterbury District Health Board on 17 July 2015 requesting the following information. 
 
  How many patients in the last 15 years have been misdiagnosed resulting in their death?   Or 
otherwise had been prevented with better medical advice from this hospital in particular  
 
I am declining your request for this information under section 18(f) of the Official Information Act 1982 
as the information requested cannot be made available without substantial collation or research.  
 
 Specifically we do not collate this data centrally and would have to review every individual clinical file 
for the past 15 years to obtain the information you are seeking. 
 
You are entitled to complain to an Ombudsman to seek an investigation and review of this refusal.
