I  write  in  response  to  your  Official  Information  Act  Request  transferred  to  us  by  the  Ministry  of 
Health, received on 12 November 2015.  You requested the following information: 
 
  Information relating to Auckland’s four acute mental health units, Waiatarau, Tiaho Mai, 
Te Whetu Tawera and Taharoto/He Puna Waiora.  The information required is pertaining to 
the number of failed discharges (patients discharged from hospital, then readmitted again 
within  a  two  week  period)  from  each  of  the  four  hospital.    May  I  please  have  a  monthly 
breakdown of these numbers from the period of July 2014 to July 2015, or the most recent 
data you have available going back a year 
 
The  information  provided  below  is  for  Tiaho  Mai,  Counties  Manukau  Health’s  Mental  Health
