I write in response to your Official Information Act Request dated 19 July 2017.  You requested the 
following information: 
 
Count  of  monthly  adverse  events  from  July  2006  to  June  2016  for  Counties  Manukau  District 
Health Board.  Please note that prior to 2012 the term used for adverse events was ‘serious and 
sentinel events’. 
 
CM Health has not captured the information you have requested in a central repository for the full 
time period you have requested.  We have been able to provide a monthly count of events from 
July 2009.   
 
Please note: 
  CM Health, in conjunction with the Health Quality and Safety Commission (HQSC), releases an 
annual  serious  adverse  event  report  which  is  available  on  our  website.    Please  refer  to  this 
information for official, validated information on serious adverse events that have occurred at 
CM 
Health.

  The information provided is captured and recorded manually in a spreadsheet.  Over the years 
the  spreadsheet  has  changed,  and  accordingly  the  nature  of  the  information  captured  has 
varied somewhat. 
 
  Please note the data we have provided relates to all serious adverse events. 
 
  The information provided in the attached table has not been verified.  To do this, a manual 
review  of  each  event  would  need  to  be  undertaken  and  would  involve  a  check  of  varying 
systems and records, whether the event was notified to the HQSC, the outcome of each case, 
and  confirmed  against  the  minutes  of  the  Serious  and  Sentinel  Events  Committee  meetings.  
We  estimate  this  exercise  to  take  approximately  10‐15  minutes  per  incident,  which  would 
equate to around 70 hours. 
 
  CM Health has a policy of allowing the first five hours of staff time expended on responding to 
an OIA request to be provided free of charge.  Thereafter, costs accumulate at a rate of $38 
per half hour or part thereof.    Please advise whether you wish to proceed on this basis.
