RE Official information request CDHB 9689
I refer to your Official Information Act request dated 14 August 2017 requesting the following
information from Canterbury DHB pertaining to Mental Health.
What is the process that case managers follow to determine if someone is fit to leave their
outpatient facility? Your request was subsequently clarified on 17 August 2017 as “the criteria for
discharge from outpatient treatment”?
Case managers follow documented criteria in deciding if someone is fit to leave outpatient treatment.
The criteria is as follows:
Discharge or transfer criteria
The consumer’s ongoing needs are able to be met within a primary health care setting, or
The consumer is referred to and accepted by another SMHS service, or
The consumer withdraws from treatment and does not meet criteria for the Mental Health Act
1992, or
The consumer is unlikely to benefit from further specialist mental health treatment.
Upon meeting the criteria staff develop a discharge or transition plan in consultation with the consumer
and family, (if appropriate, general practitioner and other services are involved).
