Official Information Act (1982) Request
I write in response to your Official Information Act Request, which was logged by us on 24 May 2018.
My apologies for the initial misdirection of your request submitted to FYI.org site at 5.00pm on 24 April.
You requested the following information:
“Many of our people in the community undergo treatment for gallstones and cholecystitis. Treatment
options include elective gallbladder surgery commonly known as cholecystectomy (laparoscopic
cholecystectomy).”
Please provide a copy of all the information you provide to clients (public health consumers) during
the clinical assessment, cholecystectomy (laparoscopic) preoperative informed consent and surgery
admission process.
Information to include but not exclusive to:
• Patient treatment information handout / fact sheet brochures
• Patient informed consent form or forms
• Elective surgery consent form or forms
• Post-cholecystectomy wound care, recovery and diet information
And any other handout information provided to clients, include also if possible published and revision
dates. And any links to online published information on your website.
Please refer to the attached copies of relevant documents – these are provided to all patients for these
procedures prior to and/or during admission.
• Gallbladder Surgery patient information pamphlet
• Patient Consent form
• Health Questionnaire
• A summary of the verbal information given to patients on discharge on their post-operative care
of wound and General Practice follow-up.
Depending on circumstances, patients may also receive individual assessments and other information
from the multidisciplinary team, for example from physiotherapy, dieticians and social work.
