[FYI request #8039 email]
Dear Mr Preston
Official Information Act 1982 Request – Category Dates
Thank you for your request for information received on 15 June 2018. You requested that the
Earthquake Commission (EQC) make available to you the following information under the Official
Information Act 1982 (the Act):
‘At what date did EQC add categories for Failed Repair Strategy, Incorrect Repair Strategy and Scope
Omission on their Agreement of Work Forms?’
In response to your request, I can confirm that Fletcher Earthquake Recovery (EQR) first started
using the categories Failed Repair Strategy, Incorrect Repair Strategy and Scope Omission on their
Agreement of Works forms (AOW) on 16 October 2014. These categories were subsequently
adopted by EQC on its AOW forms. Despite a thorough search of our records, we are unable to
determine the exact date as to when EQC implemented these categories, therefore this information
is refused under section 18(e) of the Act as
the document alleged to contain the information
requested does not exist or cannot be found. This response completes your request for information. You have the right to ask the Ombudsman to
investigate and review my decision. The Ombudsman can be contacted at PO Box 10 152, Wellington
6143, or on Freephone 0800 802 602, or at
http://www.ombudsman.parliament.nz. Yours sincerely
