Thank you for contacting us with your request for proposal and providing a familiarisation of your facilities and current
operation. It is our understanding that Plant & Food Research (PFR) is seeking an investigation into the ongoing asbestos
removal project
, and in addition, the potential exposure of its staff and
contractors to airborne asbestos at the Mt Albert Road site.
INTRODUCTION

INVESTIGATION PLAN
Following our briefing on Wednesday we understand that you have concerns around the health and safety of your staff,
contractors and visitors as a result of the removal of asbestos from the building and the subsequent rediscovery of further
asbestos after clearance and sign-off, resulting in potential exposure to harmful dust.
The two objectives of this investigation are:
1. To understand the facts around the recent asbestos removal operation in order to satisfy a request from Worksafe
that you initiate your own internal investigation, and to prevent future issues of this nature.
2. To understand and clarify the involvement in the asbestos removal process by all parties / entities.
To that end we hope to provide some reassurance that PFR have responded appropriately to the potential risk and to
promote efficient progression of the project.

Our investigation plan is proposed as follows:
Phase 1
▪ Interview key personnel to establish high-level facts and determine further persons for interview
▪ Collate & review all documentation associated with asbestos removal operations
▪ Develop & publish a detailed interview plan outlining next steps for your approval
Phase 2
▪ Conduct Investigation as per agreed investigation plan
▪ Report back regularly with findings and refine focus as a result
▪ Produce comprehensive timeline of the process and gap analysis
▪ Produce an association chart of entities involved in the operation to clarify contractual relationships
Phase 3
▪ Report findings in a presentation to yourself and your team (method & recipients TBC) in two formats:
o Health & Safety related issues to satisfy Worksafe request
o Contract / liability related information
▪ Recommendations for future direction & completion of project
Our proposed methodology will be to take a holistic approach to the investigation and an integral part of this approach is to
gain a reasonable understanding of how the asbestos removal process operated in order to identify the areas where safety
may have been compromised and progress disrupted. The result of this would be factual unbiased reporting documents
that could provide a basis for future direction.
To achieve this, we will need from you:
▪ A work order to proceed
▪ A site induction in terms of Health & Safety
▪ Site access with 24/7 access to all but very restricted areas
▪ Appropriate selection of site plans
▪ A list of, and access to, key personnel for interview: H&S Manager, Site Manager, Programme Manager, Project
Manager etc.
▪ Clearance with all security personnel for on-site attendance
▪ A secure office area from which to work
▪ Access to, & copies of, existing documentation relating to asbestos operations
▪ Authority to access & copy electronic data (documents, emails) pertinent to the investigation for forensic analysis
▪ Permission to take photographs where they are appropriate to the investigation

We believe that Phase 1 could commence on Monday 12 February 2018 with the Investigation plan being published
We propose the following work flow for the investigation:
▪
Phase 1:
= $7,500 plus GST (plus reasonable disbursements such as mileage to/from site)
▪ Once Investigation Plan is agreed upon, we will be able to confirm costs for Phase 2 & 3.
CONCLUSION
We are grateful to have the chance to provide investigation services to Plant & Food Research and if we are able to gain
your approval today we will be in a position to arrange our investigator for a Monday start.
