Dear University of Otago,
The Ministry of Health publishes information about the 10 most frequent adverse reactions to the COVID-19 vaccine but doesn’t appear to publish any information about the 10 most severe adverse reactions.
What are the 10 severest reactions to the COVID-19 vaccine (excluding death) in New Zealand?
How many reports have there been of each of the 10 severest reactions to the COVID-19 vaccine (excluding death) in New Zealand?
Does CARM keep records of each individual severe reaction to the vaccine? If so, where is such information published?
Are vaccine recipients provided with any information about the 10 severest reactions before they are vaccinated?

We refer to you information request dated 21 May 2021 made under the Official Information Act (Act).
As the information you have requested is believed to be held by, and more closely related to the functions of the Ministry of Health, we are transferring your request in full to that agency for response, in accordance with section 14(b) of the Act.
The Ministry of Health holds all the information relating to Covid-19 vaccinations and the database is with them. Information regarding the number of vaccinations being administered and a summary of adverse events is being collated and published by the Ministry of Health on a weekly basis.

The weekly updates of the adverse events is at the bottom of this webpage.
All cases of Adverse Events which are being reported via the CARM online system are being held in a Ministry of Health Covid-19 database along with the cases reported via the Vaccination Centres. These cases are being collated by the Ministry of Health Covid-19 team.

As required under the Official Information Act 1982, the Ministry will
endeavour to respond to your request no later than 25 JUNE 2021, being 20
working days after the day your request was received.
