1. Please provide me with all information between the Department of Corrections and the New Zealand Medical Council occurring since 1 December 2020 and the date of this request, concerning Dr Deborah Waxman. This is to include but not be limited to any file notes of telephone conversations held between the parties. This is also to include but not be limited to that originating from/received by Dr Damien Tomic, Auckland Prison staff, and Mount Eden Prison Staff.
2. In addition, please provide all information held by Department of Corrections since 1 December 2020 to the date of this request, concerning Dr Deborah Waxman, including but not limited any enquiries, correspondence or contact with/from New Zealand Police.
