Dear Ministry of Health,
Based on the Pilot Program mentioned in the minutes dated 13th Aug 2021(page69)That large supermarkets were participating in a Pilot Program for covid-19 vaccination in the workplace.
Requesting information on exactly which supermarkets took part in the workplace pilot program,Please specify the name of the supermarkets,Location,towns,suburbs in New Zealand,also what Date the Pilot program commenced.Which health companies / health providers were involved in implementing the Pilot Program in the workplace.What financial incentives/or penalties were given to enforce that supermarkets took part in the Pilot Program,please name all supermarkets.Also Minutes of the meeting between Fiona Mitchell and any other Ministry of Health staff involved in the Workplace Vaccination Pilot Program.

Health NZ needs more time to make a decision on your request.
The Act requires that we advise you of our decision on your request no
later than 20 working days after the day we received your request.
Unfortunately, it will not be possible to meet that time limit and we are
