Dear Health New Zealand,
Electronic Patient Report Form from Ambulances to DHBs and the vaccination history

For the years 2020, 2021, 2022, 2023 I request the following information:
I request the total EPRF number records by year from ambulance providers to hospitals.
For each year, I would like the total number of EPRF records received that contain the word "vaccination" or "vaccinated" in the patient's history field.
Kindly provide this in readable PDF or CSV format.

There should be reporting capabilities on ePRF from one of the various reporting systems at the MOH, eg clinical reporting tools.

Thank you for your follow up query regarding Electronic Patient Record
Forms on 27 January 2025. Set out below is a more detailed explanation as
to why we are unable to provide the information in response to your
request.
The database for the ePRF (Electronic Patient Record Forms) is not held by
Health New Zealand. The data is owned by Hato Hone St John (via their
contracted service provider) and Wellington Free Ambulance.
The ePRF data that is held by the Health NZ districts, i.e accessible by
Hospital staff, is not available for summary extraction of the requested
information. Health NZ does not have a centralised data repository that
holds this information. Health NZ would need to manually search both
digital and hardcopy records across its 18 districts to gather the
information you request (if held in an accessible format) and collate the
response.
As you will be aware, our district kaimahi are focused on supporting our
core mahi of caring for patients, be this in specialist or support roles.
We consider that diversion of these kaimahi to complete the tasks required
to provide the information would impact on Health NZ’s core work.
This information may be held by Hato Hone St John in its centralised
database. We invite you to reach out to Hato Hone St John, who may be able
provide you with information. Please note they are under no obligation to
provide the information as they are not required to by law.
