Re: Two incidents at Glenfield pools
Thank you for your email dated 12 November 2024, in which you requested information
relating to notifications to WorkSafe of two incidents at Glenfield Pools.
The information I’m June requesting relates to the following two incidents at Glenfield
pools.
i) 23 June 2020 - near drowning of Mr Hamish Jamieson, after 7mins and 29secs
underwater, before being dragged from the pool, he had been starved of oxygen
and his heart had stopped.
ii) 10 August 2020 - A woman died at the pools, Police were involved, and the
centre was closed for two days.
Worksafe class the above incidents as serious harm and must be notified to Worksafe
without delay.
- Please provide a copy of the notifications sent to Worksafe for each of the above
incidents.
- If Worksafe were not notified of either or both of the incidents, please explain why?
Please find attached the WorkSafe notification for the incident on 23 June 2020 which was
submit ed electronically by the Risk and Assurance Manager, and confirmation from WorkSafe
of a telephone notification from the Corporate Health and Safety Advisor of the incident on 10
August 2020.
