Dear Auckland Council,
The information I'am requesting relates to the following two incidents at Glenfield pools;
i) 23 June 2020 - near drowning of Mr Hamish Jamieson, after 7mins and 29secs underwater, before being dragged from the pool, he had been starved of oxygen and his heart had stopped.
ii) 10 August 2020 - A woman died at the pools, Police were involved and the centre was closed for two days.
Worksafe class the above incidents as serious harm and must be notified to Worksafe without delay.
- Please provide a copy of the notifications sent to Worksafe for each of the above incidents.
- If Worksafe were not notified of either or both of the incidents, please explain why?

Dear Ms Pillay,
Thank you for your response and the information relating the the death of JI Suqing at Glenfield Pools on 10 August 2020.
The information you provided about the incident involving Hamish Jamieson at Glenfield Pools on 23 June 2020 appears to be an irregular notification to Worksafe, ( the normal procedure would be via a phone call ) the document provided does not say by what means Worksafe was notified or the specific date and time. Therefore In the absence of these details which would confirm Worksafe had been advised, then please provide a copy of Worksafe’s letter of acknowledgement, this would also include there reference information.
This is not a new OIA request, it is still within the scope of my original request which asked for a copy of the;
- Worksafe notification, or
- If Worksafe were not notified please provide the reasons why.
I trust you will provide the Worksafe acknowledgement or an explanation why Worksafe were not informed.

Thank you for your email. Apologies, we had missed an attachment to your
response.
The attached correspondence shows a member of our staff initially rang
Worksafe and was not able to reach his primary contact for notifying
incidents, so he left a message and completed the online notification
tool. Worksafe called back the following day.
Below is a brief timeline of events for the notification.
o Incident: Tuesday 23 June 2020, 8.48pm.
o Call to Worksafe (not answered) and completion of online notification
tool by John Lee: by Wednesday 24 June 2020, 2.09pm.
o Return call from Worksafe: by Thursday 25 June, 3.05pm.
o No further action from Worksafe.
