Hi Megan
Further to my conversation with Graham Ross and Martyn Griffiths who attended the Glenfield
rec centre today to review the incident with staff where a young man became unconscious
underwater and was taken to Auckland City Hospital I have filled in the Worksafe Online
Notification tool (printout attached) and rang and left a message for Justin Michels our primary
Worksafe contact. I am waiting for Justin to return my call as he was unavailable to speak.
As an update, we have been told by the hospital that the young man concerned is now in a
serious but stable condition. We have spoken to his family who have, I am told, been responsive
to our engagement so far.
It appears from viewing the CCTV footage that the young man was practicing holding his breath
underwater water and lost consciousness. He floated to the surface where he was spotted by
our lifeguard who retrieved him and began CPR, then an AED utilized and by this time St John
Ambulance had arrived on scene and transported him to the Auckland City Hospital.
I will update you as soon as I find out more.

Afternoon all,
Justin Michels, our primary Worksafe contact returned my call just now. We had a good
conversation about the Glenfield pool incident and I came away from the call feeling reasonably
comfortable.
As you are aware, we completed the online Worksafe notification template yesterday afternoon
which I copied to Megan and Bron. The notification goes through a triage process at Worksafe
which decides the path it takes and whether or not they will do their own investigation or
whether they will simply touch base and check they are happy with the investigation conducted
by Council.
I spent some time going through the incident details (as I know them) with Justin and he feels we
have followed the right processes and in a timely and organised manner. He was find with the
fact we did not freeze the scene – would not achieve much was his comment. Justin did not
commit to which triage path would be followed but I got the feeling it would be the path where
Worksafe will want us to share our findings which they would then review with us. Justin did say
he would expect some robust outcomes from our investigation and not simply “train more
people” or other administrative controls, more PPE or other “light touches”. Some food for
thought.
Anyway, once the notification has been triaged it will come across Justin’s desk and he will be in
touch.

In the swimming pool at the Auckland Council recreation centre.
How did the event happen?
The young man was practicing holding his breath under water in the swimming
pool. It is thought he became unconscious underwater then floated to the surface
where he was spotted by the pool lifeguard.

The swimming pool is an inside facility not exposed to the elements. The centre is
well lit. It was supervised by experienced life guard who was on duty at the time of
the incident.
