3) Professional Indemnity/Liability and /or Directors' and Officers' Liability Insurance:
(a)Name(s) of insurer(s); (b) Limit(s) of liability; (c) Full copy of policy wording.

4) Personal Bonding for employees: (a)name(s) of insurer(s); (b) Limit(s) of liability; (c)Full copy of policy wording.
