May I seek a copy of the appointment letter sent to the appointee (one example is sufficient) , feel free to redact any personal information. Also do you issue warrant card to the appointee, if so, an example of the warrant card would be much appreciated.

Also, I would like to request the job description of the Director of Area Addiction Services.
