Dear Sir
I am writing to make an official information request under the Official Information Act 1982.
Dr Peter Bushnell, Chairman of WellStop Inc. an NGO that holds contracts with the Ministry. In an email communication to me dated 28 August 2015, Dr Bushnell has made claims that:
“Further I have no doubt that WellStop cares for clients and takes appropriate steps to ensure client safety. Recent audits by CYF have indicated that WellStop meets all standards for both care of staff and care of clients, including appropriate reporting to CYF.”
My request is for information about the nature and results of the audits the Ministry of Social Development conducts on WellStop’s processes and performance against their contract deliverables on both the ‘Early Intervention’ and ‘Youth’ contracts the Ministry holds with WellStop Inc.
The information I am requesting is as follows:
1. When was WellStop last audited by the Ministry and what period of time did this audit cover?
2. When will WellStop next be audited by the Ministry and what period of time will this audit cover?
3. What are WellStop’s contract deliverables and service standards for both their ‘Early Intervention’ and ‘Youth’ contracts?
4. What was WellStop’s performance against those contract deliverables and service standards at their last audit?
5. What WellStop documentation is reviewed as part of the audits on both these contracts?
6. Is WellStop’s ‘Critical Incident Report Register’ reviewed as part of the audits on both these contracts?
7. Is WellStop’s ‘Notifications to CYF Register’ reviewed as part of the audit on both these contracts?
8. What, if any, other processes do the Ministry utilise to satisfy itself that WellStop Inc. takes actions that appropriately identify and manage risk for both the clients WellStop Inc. is contracted to provide service to, and for the staff that provide those services on the ‘Early Intervention’ and ‘Youth’ contracts?
Given I am requesting this information because I remain concerned about a child’s safety, and the safety of WellStop Inc. staff working with clients funded through the ‘Early Intervention’ and ‘Youth’ contracts, could you please treat my request for this information as urgent, and expedite your response.
