I have recently come across an ACC report commonly referred to as the "Riley Report" which is an Executive Leadership Team Issues Paper dated 16 February 2009 authored by ACC's Phil Riley who at that stage was the Long-term Claims Project Lead on the Strategy for the Future Management of Long-terms Claims.
Ms Denise Cosgrove, the then ACC General Manager, People and Business Services, was associated with and named on the front page of this Executive Leadership Team Issues Paper.
THE AGE Victoria, at http://www.theage.com.au/victoria/worksa... published an article by Farrah Tomazin and Vernon Small on October 21 2012. The article made the following comments;
"A SENIOR bureaucrat linked to one of the biggest privacy breaches in New Zealand’s history has been appointed as the new boss of Victoria’s key workplace health and safety body.
The Baillieu government has quietly appointed Denise Cosgrove, a general manager at New Zealand’s Accident Compensation Corporation, as chief executive of WorkSafe Victoria."
......"Ms Cosgrove was also caught out providing misleading information — which she described as ‘‘technically correct’’ but ‘‘cute’’ — denying the ACC gagged its rehabilitation providers from criticising it. Evidence subsequently emerged they had been forced to sign confidentiality clauses,
She was also criticised for detailing a strategy of cutting long-term clients from the books at a conference in Australia last year, describing them as ‘‘low-hanging fruit’’.
I am writing to establish under the Official Information Act as to whether Fairway Resolution Limited's, Service Delivery Team member Tanya Cosgrove is in any way related to ex ACC General Manager Denise Cosgrove, and if so, it would be appreciated if you could advise under the Official Information Act as to the nature of their relatedness.

I am writing in reply to Mr Greg Pollocks written response dated 31 August 2016, received via FYI on 1 September 2016, to my OIA request of 5 August 2016.
Having recently reviewed Mr Greg Pollock's answer which read saliently;
"With reference to section 9(2)(a) of the Official Information Act 1982, we note that there are privacy implications to your request, and we have considered protection of the privacy of the individuals concerned. However, we confirm that as far as we are aware, there is no relationship between the individuals you have referred to."
I note though that Mr Pollock's response does not provide me with a definitive yes or no....and links his response to this ambiguity by stating "as far as we are aware"....which infers that neither Mr Pollock, nor any other staff member at FairWay Resolution Limited has asked the question of Tanya Cosgrove, as to weather Tanya, is in any way related to Denise Cosgrove, ex ACC employee that I refer to in my OIA request of 5 August 2016.
I respectfully request under section 23 of the Official Information Act 1982 for confirmation from Mr Greg Pollock in writing if he or any other person at FairWay Resolution Limited, prior to having provided me with his written response letter dated 31 August 2016, had actually directly asked Tanya Cosgrove if she was related to Denise Cosgrove.
If nobody within FairWay Resolution had directly asked Tanya Cosgrove if she is related to Denise Cosgrove, prior to Mr Pollock providing his written response dated 31 August 2016, it would be appreciated, in providing further clarity to Mr Pollock's ambiguous response of 31 August 2016, if Mr Greg Pollock could establish definitively from enquiring with Tanya Cosgrove if she is related to Denise Cosgrove, and if Tanya is, confirming to me under the OIA a1982, as to whether they are related and if so, how they are related.
