Dear MidCentral District Health Board,
I would like to request following information under official information act:
1. Count of monthly adverse events from July 2006 to June 2016 for MidCentral District Health Board. Please note that prior to 2012 the term used for adverse events was 'serious and sentinel events'.

Also, I do have yearly count of adverse events, but for my research purpose I require monthly events.

Dear Muriel Hancock,
I refer to your email dated 28 August 2017 - relating to transfer of official information request to HQSC.
Please note that I have been in touch with HQSC for data on adverse events. They have advised me that they only hold yearly count of adverse events, which is insufficient. Furthermore HQSC advised "We have not collected or analysed the numbers of adverse events by month".
Please withdraw the official information request transfer to HQSC as it is an unnecessary use of HQSC's resources. As a DHB, it is much more efficient for MidCentral District Health Board to provide all the monthly count directly to me. Furthermore, please note the request I made https://fyi.org.nz/list/successful?utf8=... to other DHB's has been successfully processed including much smaller DHBs comparable to Midcentral like Lakes.
Please reconsider my request again otherwise I will be forced to escalate to Kathryn Cook or/and Ombudsman.
