Dear Accident Compensation Corporation,
I refer you to Phil Riley's ACC's Executive Leadership Team Issues Paper to ACC's Executive Leadership Team dated 16 February 2009, in particular paragraph 9.2 on page 12 which reads and I quote;
"9.2 ACC currently contracts out the management of a small number (250) of LTWC in Auckland to it's subsidiary, Catalyst Risk. Extending this, and/or outsourcing to private sector claims managers may be considered if required.".
OIA Request 1:
Please advise the date that Catalyst Risk was set up in as an ACC subsidiary for the purpose of ACC contracting out the management of LTWC.
OIA Request 2:
Please provide copies of all official information associated with the formation of Catalyst Risk, as a subsidiary of ACC for the management of LTWC in Auckland inclusive of the business plan, structure and number of employees and position description's along with the organisational chart that shows Catalyst Risks Job Structures and Job descriptions.
OIA Request 3:
Please advise how many staff were initially employed in ACC's subsidiary Catalyst Risk.
OIA Request 4:
In relation to ACC's answer to my OIA Request 3 above, please confirm how many previously employed ACC staff obtained/filled/ or gained transferred/seconded employment with Catalyst Risk when Catalyst was first set up as an ACC subsidiary.
OIA Request 5:
Does Catalyst Risk, or any other contractor still provide contracted services to ACC for the management of LTWC, or has this relationship ceased? If it has ceased, please confirm the month and year that this subsidiary relationship end.
OIA Request 6:
Please provide me with copies of the pay/renumeration scale at the time Catalyst Risk was first set up as an ACC Subsidiary that applied to;
a) ACC staff that worked with ACC claimants that were categorised in the ACC LTWC stream.
b) Catalyst Risk staff that were first employed with Cataylst Risk when it was first set up as an ACC subsidary for the purpose of managing clients in the LTWC stream.
OIA Request 7
Please confirm whether Catalyst Risk employees initial pay/renumeration subject to any additional financial reward based on the number of LTWC that were exited from the LTWC stream by the employee. If so please provide the policy outlining the way the additional remuneration was calculated.
OIA Request 8
Please confirm whether employees of Catalyst Risk, at any stage that Catalyst Risk operated, were provided with financial incentive/reward additional to their base salary, based on the number of clients that they exited from the LTWC for ACC through Catalyst Risk. If so please provide a full copy of the policy surrounding the calculations of how the additional pay incentives were calculated.
OIA Request 9
Please confirm whether Ms Leanne McDonnell was ever employed and or contracted to ACC's subsidary Catalyst Risk, or any of it's previous or successive iterations of name and or title, along with providing the additional following official information if Ms McDonnell did work for Catalyst Risk;
a) The date that Ms Leanne McDonnell first started with Catalyst Risk, and
b) copies of all job roles and position descriptions that Ms Leanne McDonnell was remunerated for at Catalyst Risk.
c) the date that Ms Leanne McDonnell ceased to work for Catalyst Risk
d) The date that Ms Leanne McDonnell gained employment with ACC, for which she ended up case managing my claims in 2014.
e) copies of all job roles and position descriptions that Ms Leanne McDonnell was remunerated for at ACC, and specifically include that were in force between the period 1 January 2014 and present.
f) Please provide policy copies of Ms Leanne McDonnell's performance expectations and financial rewards that were associated with the exiting of LTWC clients.
OIA Request 10
During 1 January 2014 and 1 September 2015, please advise whether Ms Leanne McDonnell worked solely at ACC's Henderson Branch, or whether Ms Leanne McDonnell provided services to more than one ACC branch within the Auckland Area, additional to the services is provided to Henderson.

Please provide me with copies of Ms Leanne McDonnell's job roles/job descriptions, remuneration/pay range and the criteria for calculation's for performance based bonuses based on reducing the corporations LTWC claimants through exiting them, for job roles/positions held within ACC between the period 1 January 2014 and 1 September 2015

It would be appreciated if you could confirm whether ACC ever held a similar service contract with AON NZ LTD (Also known as AON Risk Services Ltd - Also known as AON Risk Management Ltd - Also known as WorkAON) for the management of ACC claimants in the LTWC category. If so when did this start and when id it cease?

I advise that my interest in the official information that I requested on 13 June 2018 through FYI.og.nz titled "Request for information in relation to Catalyst Risk, ACC and ACC case manager Ms Leanne McDonnell" is as follows;
I have an interest in insurances having worked previously in the field, albeit in personal and commercial lines, and was involved in underwriting ACC lines when ACC risk was open up to the commercial insurance market.
Currently requiring the assistance of ACC on a longer term basis since 2012, I have an interest in how the systems and processes ACC engaged prior to me getting injured, and has engaged since have shaped the provision of ACC services to me and other ACC clients,in particular clients of ACC whose injuries have not healed, or require further assessment for comorbid injuries not properly assessed.
Catalyst, and possibly AON appear to have shaped the environment to an extent that ACC's Phil Riley and Denise Cosgrove set about modeling ACC internally as part of their strategy for the case management of the Corporation's LTWC stream. I am interested to establish whether Catalyst introduced performance based commercial financial incentives
I am interested in establishing how Catalyst was set up, and to see if the this was the environment where the concept of the renumeration for performance bonuses for exiting LTWC claims was incubated prior to the introduction to use at a later date within the Corporation itself, together with how the ACC subsidiary was structured.
In terms of my interest in regard to Ms Leanne McDonnell's past experience with ACC and ACC Subsidiaries, job roles, performance expectations/rewards and case management experience with the LTWC stream, Ms McDonnell took over my case management in Early 2014, and I would like to be provided with the official information that I requested in association with ACC to determine the length of time that Ms Leanne McDonnell has been involved in the case management of long term workers compensation streams. I have read in the past that Ms McDonnell was employed with both Catalyst managing LTWC claims and AON, but the source was not official and therefore the claims cannot be verified.
Catalyst was set up by ACC to target long term claimants as a business model to exit claimants to reduce the Corporation's liability, and I would like to independently determine upon receipt of the Official Information that I am requesting whether Ms McDonnell, when later being reabsorbed back into ACC from Catalyst retained a similar role of working specifically with Longer Term Workers Compensation clients in her role, and whether that role applied between 1 Jan 2014 and 1 Jan 2016.
I trust that my expressed interest satisfies your imposed interest test, and I look forward to receiving the full information as requested on the 13 June 2018, pursuant to the time provisions of the OIA from 13 June 2018.
