Thank you for your request under the Official Information Act 1982 (the
Act) to the Ministry of Health (the Ministry) on 22 October 2021 for:

 

“I would like to know how can I apply for a vaccine exemption certificate
based medical grounds, religious grounds and any other lawful grounds.
What is the current process? Please supply the current process and
procedure, along with any relevant forms.

 

As per your record how many of these exemptions have been issued.”

 

A religious objection to the COVID-19 vaccine is not an approved exemption
from immunisation. The process to apply for a temporary medical exemption
can be found at:

Vaccinations are currently only mandatory for those individuals in the
health and disability, education, and correctional workforces. Further
