Please treat this request as urgent. My reasons for requesting this be treated as urgent (s12(3) of the Act requiring the same.) This request relates to a currently active outbreak of the Delta variant of the Covid-19 virus. The information requested specifically relates to how this outbreak is being managed within the CMDHB area. The information requested directly identifies situations where a significant risk of onward transmission results from the currently employed case management approach. I am a New Zealand citizen. I make the following request under the Official Information Act.

I request the following information in relation to known positive cases which have been, or are currently being, managed 'at home' by the CMDHB Public Health Unit.

1. The number of instances where a positive case, isolating at home, is known or is suspected to have left their home while under isolation.
2. The number of instances where a positive case, isolating at home, is known or is suspected to have received visitors to their home while under isolation.
3. The number of instances where a breach or suspected breach of requirements by a positive case to stay home and/or to not receive visitors, has required 'Contact Tracing' of contacts made as a result of that breach.
4. The number of instances where a breach or suspected breach of requirements by a positive case to stay home and/or to not receive visitors has resulted in or is suspected to have resulted in the onward transmission of Covid-19.
