Informed Consent #4 - Deaths due to Pfizer vaccine

Dear Ministry of Health,
As of today, 23 November 2021, there have been 40 deaths attributed to Covid-19.
I understand many of those people had an existing comorbidity that could have caused death with or without Covid-19.
For example the man who died from a gunshot wound has been classified as a Covid-19 death even though his comorbidity was death.
Currently Medsafe’s CARM webpage shows 97 deaths occurring after taking the Pfizer vaccine up to and including 30 October 2021.
Of those 97 deaths only one has been confirmed as being likely due to vaccine induced myocarditis.
My questions please:
1) How can the Ministry of Health make a determination on a death caused by Covid-19 within what appears to be hours before the next propaganda broadcast when they cannot determine the likely cause of death of 96 people since 20 February 2021?
2) Death is not shown as an adverse effect on the official Govt Pfizer Comirnaty Data Sheet even though the Govt is mandating vaccinations. Under what legal provision can the NZ Govt deny disclosure of risks and choice of alternatives for a medical treatment please?

Thank you for your requests under the Official Information Act 1982 (the
OIA) on 18 and 23 November 2021 for information regarding the COVID-19
Pfizer vaccine.
Each part of your request is responded to below:
On this basis I don't believe I've been given informed consent. You
don't show death as an adverse reaction - is this because it's
considered a result of an adverse reaction?
For your clarification, the Ministry of Health does not give informed
consent. Individuals may consent to be vaccinated.
There has been one confirmed linked death to the COVID-19 vaccine in New
Zealand. Information regarding deaths is included in the safety reports
you were

Additionally, any deaths are recorded as an outcome, not a reaction.
Could I please see a picture of the Pfizer phials used in NZ.

How can the Ministry of Health make a determination on a death caused by
Covid-19 within what appears to be hours before the next propaganda

While the Act allows New Zealanders to ask for information from Ministers
and government agencies, there is no requirement for agencies to create
new information, compile information they do not hold or provide or prove
an opinion. Your question and the statement to support it appear designed
to engage in a debate about the Government’s COVID-19 vaccination
programme, rather than a request for official information. The Act does
not support requests where an opinion, comment, argument, or hypothetical
statement is put to the Ministry for response, couched as a request for
information. This question is therefore refused under section 18(g) of the
Act on the grounds that the information sought is not held by the
Ministry.
No determination has been made by the Ministry on causes of death. This is
for the Coroner to determine. Medsafe only determines the probability that
vaccination may have caused a reaction that could have resulted in death.
Information about the follow-up process for reports of significant adverse
events with the COVID-19 vaccines, including fatal reports, is publicly

As stated previously, any deaths are recorded as an outcome, not a
reaction. It is the job of a healthcare professional to discuss choice or
risk with a consumer. Under the Code of Rights, healthcare professionals
must discuss this. Information about the Code of Rights is available

In addition, the data sheet and and consumer medicine information (CMI)
is publicly available to allow access for everyone to find the risks and
alternatives for themselves. The data sheet and CMI for the Pfizer
