This is a request for information from to the Office of the Coroner which comes under the Ministry of Justice.
The New Zealand Herald reported a death on Friday 10th September 2021 of a year 13 teenage student who had received a covid vaccination.

"The teenager died on Friday...The country's' top coroner has this morning reiterated that the death of an Auckland teenager is not related to the Covid-19 vaccine.
Chief coroner Judge Deborah Marshall said this morning that this was not the case."
The article's date 15th Sept 2021 which is a few days after the death of this teenager.
Kindly share for this death on 10 Sep 2021 for this teenager:
1. the date that the Coroner's investigation began;
2. the date that the Coroner's report was concluded.
3. Please include the dates for Coronial Autopsy Report and
4. the date of concluding Principal Pathological Findings.

I have a new request for information very much linked to this one which is as follows:
The press statement dated 15 Sep 2021 from Chief coroner Judge Deborah Marshall said that the cause of death was not related to the vaccine prior to the coroner's formal investigation.
From the 10th Sept 2021 - 15 Sept 2021:
Kindly share all memos, emails, text messages and other online/digital correspondence (eg whatsapp, MSTeams) between Chief coroner Judge Deborah Marshall , her staff between:
-the Department of the Prime Minister's Office,
-Jacinda Ardern,
-The CV TAG
-and any Ministry of Health staff including Medsafe.
The topics for the above correspondence media are:
Comirnaty, and/or
the Pfizer vaccine, and/or
Covid-19 and/or
the teenage girl in question and/or
thrombosis and/or
adverse event.
