Dear Canterbury District Health Board,
In response to a recent OIA request made via FYI, the Canterbury DHB explained that staff receiving hospitality had to fill out a declaration form. A request for specific information derived from these forms was refused as requiring substantial collation and research.
I would like to request the following information under the OIA:
* copies of all staff hospitality/conflict of interest forms submitted during 2014.
This should remove the need for substantial collation and research to look for specific data. If merely extracting and scanning the forms would require that, then I am happy to discuss a more limited timeframe.
I would prefer to receive an electronic response. Queries about
this request will be automatically forwarded to me by the
fyi.org.nz website.
With regards to s12 of the OIA, I am an NZ citizen and in NZ.

Dear Kathleen Smitheram,
Thankyou for your response. In order to assess whether I am appealing this to the Ombudsman, roughly how many conflict of interest forms are involved? I am assuming you counted them or got an estimate before deciding that it was "substantial".

As per our original response dated 11 June 2015, all such staff declarations are held at a Head of Department/Service Manager level and is not held in a central repository. We are therefore unable to provide the detail you have asked for without further research and collation which in our estimation would take in excess of 20 hours. You may, under section 28(3) of the Act, seek an investigation and review of our decision from the Ombudsman
