Subject: Official Information request - Uptake of Gardasil vaccine at Kingsway School, Silverdale.

Dear Waitemata District Health Board,

I am writing in regard to the recent Gardasil vaccination of girls and boys at Kingsway School, Silverdale, Hibiscus Coast in 2017.

I would like to know the percentage of girls and boys that were vaccinated this year, e.g. 20 out of a class of 30 eligible boys/girls, or whatever the numbers are. I understand you keep records of these vaccinations, as well as paper copies of the parents' consent forms.
