Request for the provision of information with respect to ACC's agreement for services between the Accident Compensation Corporation and New Lynn Physiotherapy

Dear Accident Compensation Corporation,
I am writing to respectfully request the provision of the following official information;
OIA Request 1.
Please supply me with a full PDF copy of the original signed Agreement for services documents between the Accident Compensation Corporation and New Lynn Physiotherapy, and or Nicola Collin's (Physiotherapy Registration Number 70-04909), that include the full Organisational terms and conditions along with the General Terms and conditions that applied to the physiotherapy practice/principal physiotherapist between the period 5 March 2012 and 5 May 2012.
OIA Request 2.
Please supply me with a full PDF copy of the original signed Agreement for services documents between the Accident Compensation Corporation and New Lynn Physiotherapy, and or Nicola Collin's (Physiotherapy Registration Number 70-04909), that include the full Organisational terms and conditions along with the General Terms and conditions that applied to the physiotherapy practice/principal physiotherapist between the period 14 May 2017 through to present.
OIA Request 3.
In the event that the Agreement for services documents between ACC and New Lynn Physiotherapy are held and signed under another named agency, other than New Lynn Physiotherapy, and or Nicola Collins, please confirm what the full name is that appears within the service contract, and then provide the full official information that I am seeking in terms of OIA Request 1, and OIA Request 2, above based on the named agency listed in OIA Request 3.
I am happy to have the above official information emailed directly to my email address which is [email address], and would appreciate the application of urgency in this instance, respectfully requesting that this information is made available within 15 working days from the date of this request, being by the end of business close on 4th December 2017.
The practices New Lynn offices of New Lynn Physiotherapy are noted as being located at 28 Delta Avenue, New Lynn, Auckland.
In satisfying section 12 (3) of the OIA 1982, I advise that I require this information for the purposes of seeking remedial action from both ACC and a treatment provider in relation to outstanding and unresolved matters concerning claims for covers and entitlements which were not registered correctly by and or with the Corporation at the time of treatment, nor at the time when I sought redress for the matters concerned.
The provision of this information will provide transparency and clarity with respect to the Corporations and the treatment providers obligations to me at both these junctures in my treatment/requests for remedies timeline, and should satisfactory resolution not result, this information will be required for the purposes of seeking mediation through both the Corporation's and the treatment provider's professional and regulatory association's.

It is noted that I have not received the official information that I requested from the Corporation on the November 14, 2017, as promised by ACC's Government Engagement and Support on the 17 November 2017 to be provided to me by the 12 December 2017, nor have I been provided the reason as to why this is the case.
