Dear The Treasury,
According to the Memorandum of Understanding between the Treasury and the Ministry of Health (https://treasury.govt.nz/sites/default/f...), the Ministry of Health committed to provide the Treasury financial and supplementary information of each DHB in accordance with Annex A.
If possible, could you please provide the annual information of each DHB that provided by Ministry of Health (using Annex A) from 2013 to 2017.

later than 20 December 2018, being 20 working days after the day your

If we are unable to respond to your request within this time frame, we
will notify you of an extension of that time frame.
