Dear Accident Compensation Corporation,

In accordance with the Official Information Act please provide the information requested below.

For each of the following ACC contracted assessors:

1. Dr Tina Page
2. Dr Jan Reeves
3. Dr Rosy Fenwicke.
4. Dr Phillipa Campbell

a. How much ACC has paid each of the above assessors (in $50,000 bands) for each year for the last 5 years.

b. How many ACC clients were referred to each of the above
assessors ( in bands of 50 clients) for each of the last 5 years.

c. Were extra payments made to any of these assessors for any amounts related to the report provided (ie ACC has declined the claimant based on the report) after the ONE initial invoiced report payment?
