Since January 2019 I have been seeking to read and review the full report File Ref: 19/00049.
Bill Lawson made the referral for this work related accident to be investigated by Worksafe on 6/9/18.
The investigation was completed by Andrew Sabin-Hope, Health and Safety Inspector - Investigations Specialist Interventions, 1254 Fenton Street, Rotorua, 3010

Andrew has since informed me he no longer works in Rotorua.

On liaising with Glen from the Privacy Commissioner earlier this week.
He advised me to make an official request under the Information Act.
