According to section 20 of the Health and Disability Commissioner Act 1994, everyone has the right to be fully informed before choosing a health care procedure. The Act states that “no health care procedure shall be carried out without informed consent”.
Statistically, what is the probability of an unvaccinated person in New Zealand, aged under 50 years of age, in excellent health and with no pre-existing conditions, dying from Covid-19 in 2021?
In the year to 31 December 2020, how many people aged under 50 years of age died in New Zealand? How many of those died from Covid-19?
Please provide me with all information that recipients of the Covid-19 vaccine (who are aged under 50) are given about the risk of dying from Covid-19.
Vaccinators are informed that severe reactions to the Covid-19 vaccine may include: acute myocardial infarction, Bell’s Palsy, appendicitis, stroke, convulsions/seizures, myocarditis/pericarditis, pulmonary embolism and anaphylaxis.
Please provide me with all information that recipients of the Covid-19 vaccine are given about acute myocardial infarction, Bell’s Palsy, appendicitis, stroke, convulsions/seizures, myocarditis/pericarditis, pulmonary embolism and anaphylaxis.
Since the beginning of roll-out, how many people have reported the following serious reactions: acute myocardial infarction, Bell’s Palsy, appendicitis, stroke, convulsions/seizures, myocarditis/pericarditis, pulmonary embolism, anaphylaxis?
How many, if any, under 16-year-olds have received one or two doses of the Pfizer vaccine in New Zealand?
Please provide me with all information that vaccinators provide to children under 16 before administering the Covid-19 vaccine?
What information is given to vaccinators, as part of their online education course, in June 2021 that they were not given in April 2021?

I am interested in the changes in information provided to vaccinators about vaccinating pregnant women so I was hoping the response to this request would provide the information requested but it didn’t. Instead the response said it added some items (data, fact sheets, graphic form), to the information rather than providing the actual information itself so I have raised a request myself asking for the actual information provided to vaccinators about vaccinating pregnant ‘people’ as they now refer to them.
