I write further to your email below. Thank you for allowing me to provide
information on how to make a complaint.
The role of the Office of the Health and Disability Commissioner is to
facilitate resolution of complaints about the quality of health and
disability services provided to consumers. If you have any concerns about
care that has been provided to you or someone else and you decide to lodge
a complaint about a health or disability services provider, you may
fill-out our online complaints form at this link:

Alternatively, you may wish to get in touch with an advocate from the
Nationwide Health and Disability Advocacy Service (a separate organisation
from HDC) who will support you and try to resolve your concerns. They can
be reached at 0800 555 050.
I've also enclosed brochures about HDC and the Advocacy Service for your
information.
