Dear Ministry of Health,

Could I please request any information that shows what is the care in the community model, who it involves (or involved), how much has been spent within it and how success has been measured on the outcomes and payments made.

I’m interested in anything that:

- shows the structure of what it is and how it worked, the outcomes that were expected, which may include changes over time, and particularly how care in the community was organised nationally and regionally

- this structure includes, as applicable, any organisations or providers or vendors that were or are involved, their roles within the model and the amounts they were paid - if this request is to difficult is it possible to ask for what they were paid?

- the above point includes gp payments and work expectations for that payment - how were gps involved and what where the expectations on them to provide care and what was the payment structure to them?

- what metrics were tracked or monitored to show on the delivery of care in the community?

I’m particularly interested to see what was developed to track how it delivered on its outcomes against payments and delivery, how was success tracked?

Please note I submitted this originally through email on Feb 1 2022 - I had no acknowledgment to that request nor a follow up 8 days later and I expected an automated response to my email. This is the same request on the assumption my email had difficulty getting through.

I've just realised i've not only not received a response to my request within a month - but i've also not received an acknowledgement or reference number that i can see - perhaps this request got waylaid or lost?
