Dear New Zealand Transport Agency,

 

Keely Puklowski;

 

we request that you release all personal & private information, from
August 2023 to November, all notes, all correspondence, all documents, all
conversations by managers, supervisors, contact call cente Kellie &
Sheldon notes that were recorded relating to the license BO247333 that
displays the name change TNGA MAREE SHULTZ, which is no longer accurate.
The DSL26 completed form, with referee details, and any decisions that
were made relating to the license for reinstatement.
