Ia requesting information in relation to a serious harm incident on 10/8/2020 resulting in death at Glenfield Pools, owned and operated by Auckland Council.

- Could you please provide a copy of each notifiable incident report submitted to Worksafe.

- Did Worksafe undertake an investigation and what was the outcome.
or
- If Worksafe did not undertake an investigation, please advise the reasons for this decision.

Should you require clarification of my request, please let me know.

We have received your two requests for Official Information.

 

We have completed a search for relevant information based on the two
incidents at Glenfield Pools, on 23/6/2020 and 10/8/2020 resulting in
death.  We have not been able to find any files related to this.

Auckland Council are mentioned in hundreds of notifications and the system
we use from this timeframe is not able to search by a specific location.

Do you have any other information that may help us in identifying these
incidents?

 

A file number is helpful, but I appreciate that you may not have this.

Please reply to this e-mail address with any other information you have
that may help us.
