Te Whatu Ora (Health NZ) for the following under the Official Information
Act 1982 (the Act): 

 

“Please provide a list of organisations (companies, societies, charities,

partnerships and trusts) that currently have provider agreements and/or
receive

funds from TWO/HNZ to provide mental health and/or addiction services in
the

Lakes district.

 

Please provide the agreement/contract documentation in each instance, with

In relation to Part Two of your request, Health NZ Lakes has identified
over 1800 pages of documents that may be in scope. While we appreciate you
