Dear Health New Zealand,

I am a New Zealand citizen and reside in New Zealand. Under the OIA, I request a copy of the following for Health New Zealand Waitaha Canterbury.

1. Number of people on the wait list for a first specialist appointment with Christchurch Hospital Neurosurgery.

2. Estimated wait time for a first specialist appointment with Christchurch Hospital Neurosurgery.

3. Number of people refused a first specialist appointment with Christchurch Hospital Neurosurgery department (or Outpatients Consult) in the past twelve months.

4. How many FTE 1.0 (Full Time Equivilant) Surgeons work in the Christchurch Hospital Neurosurgery Department.

5. How many Surgical Procedures (Operations by a Neurosurgeron) performed by Christchurch Hospital Neurosurgery department in the past twelve months.

6. How many Surgical Procedures (Operations by a Neurosurgeron) outsourced to Private Hospitals in the past twelve months paid for by Health NZ Waitaha Canterbury.
