Please note that this request should be considered separately from my urgent request for an update on dexamfetamine supply. Completion of the response to this request should not delay your response to my urgent request.
Can you please provide all information you hold on the current supply management issue for dexamfetamine. I am interested in the cause, management, mitigation and resolution of the issue.
This request includes, but is not limited to:
1) Any discussions, analysis, explanations or other information about the cause of the current non supply.
2) Any briefings to any other agencies or Ministers on the current supply issues.
3) All advice provided to doctors, pharmacists, pharmaceutical wholesalers, their professional organisations or any other medical professionals on the supply issues, guidance on alternatives, and their role in managing it.
4) Any discussions, analysis, briefings or other information about decisions made to seek to source an alternative medication.
5) Any discussions, analysis, explanations or other information about Pharmac's management of the current supply issue.
6) All correspondence with the supplier, or their representatives or agents or related parties, about the resolution to the issue, the cause of the issue, and approaches to mitigating the impact of the issue.
In addition, could you provide summary information on any previous supply issues with dexamfetamine.

Dear Victoria,
Thank you for you response. I appreciate the partial response given the extension you have needed.
For clarity I would like to state that the lack of supply issue I am requesting information about lasted until mid January 2016 (an earlier response from Pharmac said 13 January, in Wellington it was until 18 January).
The documentation you released in your partial response talked about an anticipated out of stock until mid December. I am assuming that this out of stock continued until mid January - that was our impression when trying to get prescriptions filled - and that your response will cover the entire issue.
If they were, in fact, two different issues I would like to extend my request to cover the second supply issue.
