MBIE claims of instances of unsafe building designs

Dear Ministry of Business, Innovation & Employment,
On 22 November 2017, RNZ attributed the following statement to MBIE:
“We have recently become aware of buildings being designed where the Verification Method did not provide Building Consent Authorities adequate assurances that public safety was sufficiently protected,” the ministry's Dave Robson, Manager of Building Performance and Engineering, said in a statement. The ministry said that in those cases of an unsafe design, councils made the designer rework it.
It is a serious matter to claim instances of unsafe building designs.
‘Safe’ and ‘non-compliant’ are not binary states. They are points on a spectrum.
Therefore, please provide the documentary evidence behind MBIE’s claim of ‘unsafe building designs’. This should include communications both internally and with external stakeholders such as Fire Emergency New Zealand and Building Consent Authorities.
