Request for all information regarding review of decision application and response

Response to this request is long overdue. By law Ministry of Social Development should have responded by now (details and exceptions). The requester can complain to the Ombudsman.

Dear Ministry of Social Development,
I have requested all information regarding an application I made for review of a Work and Income decision regarding an application for assistance with travel costs to attend a Court ordered medical appointment. The application for assistance with travel costs was made in November 2010, it was declined by a staff member named Lisa Kerr on the grounds that "Work and Income can not be seen to assist clients who do not abide by the law."
This is a gross breach of human rights, I had not even stood trial, just because a person is appearing before the Court does mean that they do not abide by the law, and I was subsequently found not guilty of all charges. The consequences of the decision however, were extremely serious, as is the assumption that people who have been charged but not tried by a Court are automatically "people who do not abide by the law."
Apparently the application for review was considered by the BRC in 2011. I request all information regarding the application for review of the decision, and the response of the BRC.
