Dear Ministry of Health,
I am requesting under the Official Information Act that you provide me with copies of the Covid 19 vaccination and booster authority forms that everyone was required to complete and sign before administration of each of their original vaccinations and subsequent boosters.
I am aware that there was a range of different Covid 19 vaccination and booster products previously available to everyone that are no longer available, as well as subsequently introduced and still currently available vaccination and booster products.
Should it be the case that there historically were, and currently are, different authority forms for the various different Covid 19 vaccination and booster products, I request that I be provided with copies of each of them.
My preference is to have the requested information provided through the FYI website.

decision on your request.
The Act requires that we advise you of our decision on your request no
later than 20 working days after the day we received your request.
Unfortunately, it will not be possible to meet that time limit and we are
therefore writing to notify you of an extension of the time to make our
decision, to 15 February 2024
This extension is required because the consultations necessary to make a
decision on your request are such that a proper response cannot reasonably
be made within the original time limit.

Dear hnzOIA,
To comply with the legal obligation to provide me with the information I requested, or to give me notice of an extension, Te Whata Ora was required to do either inside the 20 working day time-limit. Since this expired yesterday - Tuesday 30 January 2024 - your extension notice of today's date - Wednesday 31 January 2024 - is INVALID. In any event I don't accept there was any need for an extension because the requested information is readily available in your computer systems.
While Te Whata Ora is already in breach of the 20 working day time-limit, I am nonetheless giving you until close of business on Friday 1 February 2024 to provide me with all of the information I requested. Should this not happen then I will immediately complain to the Ombudsman.
Please note that I used the Ombudsman's online calculator to work out the date by which you needed to respond, or give notice of an extension. Refer https://www.ombudsman.parliament.nz/agen... I corrected input the request date as 11 December 2023 and the answer returned was that the 20 working day deadline fell on 30 January 2024. .

Thank you for your email. Please note that your request was transferred
to Te Whatu Ora by the Ministry of Health on 13 December 2023.
Section 15(1) of the Official Information Act 1982 (the Act) requires
government agencies to make their decision on requests "not later than 20
working days after the day on which the request is received by that
department". As Te Whatu Ora received the request on 13 December 2023
your response was due by 01 February 2024.
In this case Te Whatu Ora has extended the deadline for a response under
section 15A(1)(b) of the Act, as consultations necessary to make a
decision on the request are such that a proper response to the request
cannot reasonably be made within the original time limit. We notified you
of this before the 01 February deadline.
We are endeavouring to provide the finished response to your request as
soon as possible, and in any case no later than 15 February 2024.
