How many Health and Disability workers were issued covid fines?

Dear WorkSafe New Zealand,
I see there is a guidance document from the MoH here "Health & Disability Worker
Vaccinations Order, Guidance document for PCBUs and workers" dated 23 Oct 2021

Page 10 and 11 says about Worksafe:
"From 11.59pm on 15 November 2021, Worksafe may issue a PCBU with an infringement fee or fine of up to $1,000 for every worker found to be non compliant • NOTE: The Order also provides for heavier penalties for intentionally failing to comply with a COVID-19 order"

How many fines were issued of up to $1000 by Worksafe
How many heavier penalties were issued by Worksafe?

Of these fines, how many were successfully collected by Worksafe?
How many have gone off to the MoJ to be collected?
