1. a copy of your conflict of interest register for the board,
executive team and all others who are required to complete a conflict (or
potential conflict) of interest declaration.
2. details on how you decide who should complete a conflict of
interest declaration. Please also advise how you manage any potential
conflict of interests.
3. a copy of your gifts/hospitality register for the past year.
We are working on your request and will endeavour to respond to you no
later than 18 March 2019.
In the event an extension is required we will notify you.
Please note that, like other DHBs across New Zealand, Auckland DHB has
adopted a model of proactive disclosure whereby OIA responses are
publically available on our website approximately one month after they
have been released. If you believe that there are any special reasons why
proactive disclosure should not apply in our response to you, we will be
happy to consider this.
